Immunizations are audited by the state every year. If your child is not up-to-date or does not have a notarized exemption form, he/she will not be able to attend school until updated records are received. Linn County Public Health Clinic is available at discounted rates for poorly insured or uninsured families in need.
Immunizations are the cornerstone of Public Health in the United States. We are very fortunate to live where children do not die from vaccine preventable diseases. Please continue to vaccinate your children according to the recommendations of the American Academy of Pediatrics.
Vaccination requirements for incoming 7th and 12th grade students
Students need to have proof of a Tdap booster and Meningococcal (A,C,W,Y) before the 1st day of school. Please turn them into the middle school office.
Students need to have proof of Meningococcal (A,C,W,Y) vaccine at age 16 or older before the 1st day of school. Please turn them into the high school office.
All incoming 9th graders need to have the dental screening forms turned into the high school office before school starts.
