State of Iowa Requirements

Center Point Urbana School District follows the requirements set out by the state of Iowa. 

  • Below is a New Student & Student By Grade Matrix of what is needed
  • Please turn in all Health Documents to your student’s building Health Office.
  • For your easy navigation, we have provided links to each document.

New Students

Please contact the school office secretary and school nurse to discuss required paperwork.

Preschool

  • Up-to date Immunizations
  • Physical by any licensed practitioner

Kindergarten

  • Updated Immunization
  • Lead blood level test
  • Dental screening
    • Valid from age 3 years until 4 months after starting kindergarten.  UnityPoint Dental Clinic is available to provide services at discounted prices for families that qualify.  
  • Vision screening
    • Valid from one year before enrolling in K until 6 months after school starts
    • OPTION:  The Lion’s Club of Center Point provides free vision screening for CPU Preschoolers and Kindergarteners at the beginning of every school year.  A consent form needs to be signed and this can count towards the Vision Screening requirement.

3rd Grade

  • Vision screening
    • Valid from one year before enrolling in 3rd Grade until 6 months after school starts.

7th Grade

  • Updated Immunizations
    • Tdap & Meningococcal (A, C,W,Y) .
    • Meningococcal MUST be administered prior to the first day of school.

9th Grade

  • Dental Screening
    • UnityPoint Dental Clinic is available to provide services at discounted prices for families that qualify.

12th Grade

  • Updated Immunizations
    • Meningococcal (A, C,W,Y) second dose.
    • Meningococcal MUST be administered prior to the first day of school. 

Students in grades 7-12 involved in competitive athletics, cheerleading, and/or dance/drill team have the following requirements:

  • Concussion form
  • Health & Injury Consent to Treat form
  • Yearly Physical form
